Paliperidone palmitate (Invega Sustenna\u00ae) is an extended-release injectable\u00a0second-generation (\u201catypical\u201d) antipsychotic. Here is basic information about this medication.<\/p>\n

FDA-approved indications<\/h4>\n
    \n
  1. Treatment of schizophrenia<\/li>\n
  2. Treatment of schizoaffective disorder as monotherapy and as an adjunct to mood stabilizers or antidepressants<\/li>\n<\/ol>\n

    Dosage<\/h4>\n

    A. Schizophrenia<\/em><\/p>\n

    Initiation (IM deltoid): Administer 234 mg on day 1; 156 mg on day 8<\/p>\n

    Monthly maintenance dose (IM deltoid or gluteal): 39\u2013234 mg (administered 5 weeks after first injection)<\/p>\n

    Maximum monthly dose: 234 mg<\/p>\n

    (Recommended maintenance dose for schizophrenia is 117 mg, though some patients may benefit from higher or lower doses within available strengths)<\/p>\n

    B. Schizoaffective disorder<\/em><\/p>\n

    Initiation (IM deltoid): 234 mg on day 1; 156 mg on day 8<\/p>\n

    Monthly maintenance dose (IM deltoid or gluteal): 78\u2013234 mg (based upon tolerability and efficacy)<\/p>\n

    Maximum monthly dose: 234 mg<\/p>\n

    C)\u00a0Use in specific\u00a0population: patients with renal impairment<\/em><\/p>\n

    Mild renal impairment (creatinine clearance 50-80 mL\/min): Administer 156 mg on treatment day 1 and 117 mg one week later, both on deltoid muscle. Follow with monthly injections of 78 mg in either the deltoid or gluteal muscle.<\/p>\n

    Moderate to severe renal impairment (creatinine clearance<50 mL\/min): Invega\u00a0 Sustenna\u00ae is not recommended<\/p>\n

    Dosage forms and strengths<\/h4>\n

    Extended-release injectable suspension: 39 mg, 78 mg, 117 mg, 156 mg, and 234 mg<\/p>\n
